Erythema elevatum diutinum: A case report with atypical presentation

ABSTRACT
Erythema elevatum diutinum (EED) is a chronic cutaneous disease
of unknown origin. Clinically it is characterized by nodules and
red-violet plaques mainly affecting acral and extensor surfaces
in a symmetrical distribution. The histological hallmark of the
disease is a leukocytoclastic vasculitis, with concentric fibrin
deposits and microangiopathy at the final stage. We present
a 90-year-old female patient who presented purpuric macules
and plaques associated with intense erythema, as well as few
pruritic and painful papules. At first the patient was treated
with topical and systemic steroids as a possible case of contact
dermatitis, that worsened her symptoms. The histopathological
findings were consistent with eed. Azathioprine 50 mg was
added with an adecuate clinical response.
