ABSTRACT
The indication to perform surgery for pharmaco-resistant epilepsy in pediatric patients is widely documented and that it should ideally be performed as soon as possible. The same criterion of temporality applies to all ages, since epilepsy surgery may be more effective in patients with less chronic disease. However, the safety and efficacy of surgery for refractory epilepsy in older adults have not been discussed in such depth. We performed a bibliographic search to locate the concepts "surgery for epilepsy" and "older adults" in indexed medical journals. We found nine articles that effectively addressed these issues and all were included in this review. The bibliographic evidence available and analyzed in this review shows that the effectiveness results of epilepsy surgery in older adults are comparable or better than those of younger groups; however, as expected, there is an increased risk of complications in the group of older patients. Despite this, in selected cases, this resource should be kept in mind as the best treatment to obtain freedom from seizures in refractory epilepsy, despite advanced age.
