Central skull osteomyelitis, a complex diagnosis

ABSTRACT
Background: Central skull osteomyelitis is a rare, life-threatening complication of
necrotizing or malignant external otitis, which results in destruction of the skull base.
The imaging appearances can be misinterpreted as a malignancy, so it is important
the consideration of this diagnosis, both radiologically and clinically, supported by
the biopsy and culture trying to avoid the extended and aggressive surgical treatment
because of their risks.
Clinical cases: This paper reports the findings of the CT scan, MRI and nuclear
scan with Tc99, also we did an endoscopic biopsy and culture of the nasopharynx
and adjacent tissue in two cases and a microscopic biopsy and culture of the ear in
one of them.
Conclusions: Osteomyelitis of the central skull base generally affects men over
50 years of age, its onset is with headache and predominantly lower cranial nerves.
