Head and neck manifestations of tuberculosis

ABSTRACT
Objective: To describe the clinical, radiological and microbiological behavior of
tuberculosis infection of the head and neck, in order to achieve an earlier diagnosis
and prompt treatment.
Materials and Methods: A descriptive, retrospective, cross-sectional study that
reviewed clinical charts of all patients with diagnosis of head and neck tuberculosis in a
third level hospital in Mexico City from 2014 to 2019. Demographics, clinical presentation,
diagnostic methods, imaging studies and outcomes were described.
Results: There were 14 cases, with a predominance of female patients. In order of
frequency, the sites involved were cervical lymph nodes, ear, larynx and skull base,
only three cases had concurrent pulmonary tuberculosis. Diagnosis was confirmed
with culture or pathology results. Twelve patients had a successful response to treatment,
one is still ongoing and one lost follow-up, surgical treatment was indicated in
only two patients.
Conclusions: Tuberculosis infection in head and neck has variable and unspecific
manifestations, classically described signs and symptoms are very rare. Therefore, diagnosis
is a challenge for clinicians specially when there is an associated HIV infection.
Head and neck surgeons must keep in mind tuberculosis as a differential diagnosis in
order to achieve early diagnosis and treatment.
