Neonatal exogenous lipoid pneumonia by “cure for stomach ache”. Case report

ABSTRACT
Introduction. Cures for stomach ache enters the field of selfmedication,
which not infrequently, aggravate the clinical pictures
such as the case of gastroenteritis.
Clinical case. Infant female who attended in the last two weeks with
clinical gastroenteral, finally diagnosed as rotavirus gastroenteritis,
characterized by abdominal distention, mucosal diarrhoea and
occasionally vomiting. Initially the patient received multiple drug
treatments, subsequently she was treated with traditional medicine,
where vegetable oil associated with other herbs was administrated
to her. Subsequent to this it begins with progressive respiratory
distress, fever and cyanosis. On admission: respiratory rate of 86
bpm, peripheral capillary oxygen saturation (Spo2) of 68%, general
complaint complained of Silveman Andersen of 6 and abundant
bilateral bronchioalveolar rales in the lung fields. In the chest
radiograph we document bilateral infiltrates with air bronchogram
as finding radiopaque image (vegetable oil residual in the gastric
chamber). She was hospitalized with intensive management for 12
days, and released by improvement.
Conclusions. The diagnosis of exogenous lipoid pneumonia is
suspected in previously healthy patient with breathing difficulty,
initiating sudden unexplained by other causes and where
documented medicated oil administration.
