ABSTRACT
Testicular microlithiasis is detected by chance in scrotal ultrasounds performed due to different causes. It has been described its relation with testicular cancer and other entities. The aim of this work is to describe the clinical and imaging characteristics of a 54 years old, male patient with bilateral testicular microlithiasis, with background of controlled arterial hypertension, who went to the emergencies service due to right orchialgia. In the physical examination both testicles and the left epididymis were normal, but the right epididymis was swollen and very painful at touch. It was diagnosed acute right epididymitis and it was indicated a proper treatment. The escrotal ultrasound confirmed the volume increase in the right epididiymis and multiple microlithiasis in both testicles. Tumoral markers were negative. The patient is currently asymptomatic. In patients with testicular microlithiasis related to risk factors is indicated an annual assessment with scrotal ultrasound and tumoral markers. In the rest of the patients, frequent testicular self-examination and an annual scrotal ultrasound are adequate guidelines for evolutive follow-up.REFERENCES
