Isolation of Blastomyces dermatitidis in pleural fluid

ABSTRACT
Background: Pleural effusion is the pathological accumulation of fluid between
the visceral and parietal pleura, the clinical syndrome is integrated as the abolition of
freckles and vesicular murmur, in addition to percussion dullness. Its causes are variable
but the analysis of the macro and microscopic characteristics of the spill is very useful to
orient us towards the diagnosis. The treatment varies from conservative, toracocentesis,
closed thoracostomy and surgical management with washing and decortication, all of
the above depending on the etiology and severity of the condition. As for the infectious
etiology, fungal pathogens are rare.
Clinical case: A 49-year-old male patient in who fungus
Blastomyces dermatitidis
was isolated.
Conclusions: Infection by
Blastomyces dermatitidis can occur in any person but it
is more severe in immunocompromised, being able to affect virtually any organ being
more severe pulmonary infections and at the level of the central nervous system; the
treatment of mild to moderate forms is with itraconazole, in moderate to severe forms
the liposomal formulations of amphotericin B are preferred.
