Corruption and health 2: the challenges to improve the public health system in Guatemala

ABSTRACT
Introduction: the abuse of power for personal gain has been embedded for decades in the Guatemalan health system and all strata of society, and has become the standard operating procedure of many employees, politicians and organizations.
Objectives: analyze the challenges that the public health system faces in order to make the Ministry of Health and Social Welfare (MOH) more efficient and contribute to improve the health and wellbeing of the population.
Development: information and knowledge from the academic world are analyzed. Real-world lessons learned and experiences from operational and executive levels within the MOH are reviewed, including the implementation of technical and financial assistance by international organizations. Areas of thought, behavior and culture are identified and interventions are recommended to combat systemic and generalized inefficiency and corruption. A matrix comprising three social environments, three health system approaches, and three types of leadership is built to resolve problems.
Conclusions: the public health system in Guatemala will continue to fail and there will be no way to salvage the MOH from the prevailing debacle, unless a personal, health system and social reconstruction takes place.
