Direct and indirect impacts of COVID-19 in nursing home in São Paulo (Brazil): a comparison of mortality between 2018, 2019 and 2020

ABSTRACT
Objectives: the first outcome was to evaluate the mortality rate of the elderly in the nursing home, from January to July 2020, compared to the same period of the previous two years. The secondary outcomes were to determine the mortality rate by COVID-19, collect the number of cases of the disease and analyze the profile of individuals who died from it.
Methods: data was retrospectively collected regarding the general mortality from the months of January to July 2020 and the previous two years and formulated a cross-sectional observational study in a residential home of the elderly in the city of São Paulo.
Results: measures to restrict visits and events were implemented on March 19, and the first case was confirmed on March 29. The overall mortality rate in the residential homes has increased from May 2020, 3.5 times compared to the same month in 2018 and 1.85 times compared to 2019. In June 2020, the increase was 2.3 times greater when compared to 2018 and 1.85 times greater when compared to 2019. While in July, the increase was 1.67 times greater when compared to 2018 and 1.80 times greater when compared to 2019. Fifty-two cases of Sars-Cov-2 infection and 9 deaths were recorded.
Conclusion: the low number of deaths due to early restriction measures, as opposed to the increase in overall mortality of up to 3.5 times reflects the indirect impacts of the COVID pandemic – 19 in a long-term institution.
