Cardiac resynchronization: New horizons

ABSTRACT
Management of patients with heart failure is one of the most challenging problems of modern cardiology. Recently cardiac resynchronization therapy (CRT) has become one of the most promising way of treatment in this group of patients while added to conventional pharmacological treatment. CRT is now considered appropriate in patients with NYHA III or IV class, with LVEF ≤ 35%, LV dilatation, QRS duration ≥120 ms, who remain symptomatic despite optimal pharmacological treatment. Nevertheless, there is some evidence that also patients with diastolic heart failure as well as those with narrow QRS may benefit from CRT therapy. As the presence of inter- and intraventricular asynchrony seems to be the major determinant to predict a group of patients who respond the CRT therapy the challenge is to establish the parameters that allow us to diagnose the presence of asynchrony. In this paper we will comment on the ECG and echocardiographic features that may characterize patients with diastolic heart function and/or narrow QRS who may benefit from CRT therapy.
