Cavitated pulmonary and intestinal tuberculosis in a previously healthy patient

ABSTRACT
We describe the case of a 12-year-old female patient from Venezuela, who consulted for long-standing gastrointestinal symptoms, recent scarce respiratory symptoms and constitutional syndrome. Taking into account the clinical picture and the risk of infection by Mycobacterium tuberculosis given the condition of displacement, imaging studies were taken that showed a cavernous lesion in the right lower lobe with adjacent alveolar consolidation, mediastinal adenopathies and microbiological studies with positivity for Mycobacterium tuberculosis. Since the first sputum smear microscopy, due to gastrointestinal symptoms, a positive stool smear microscopy was requested. Colonoscopy was performed with lesions suggestive of related infectious intestinal involvement. Antiphymic management was indicated with adequate clinical response after initiation. Management was planned for 12 months given the finding of gastrointestinal involvement of possible disseminated origin. Tuberculosis is an infectious pathology that can simulate diverse clinical pictures, its etiology should be suspected in endemic areas and patients with suggestive clinical symptoms and baseline risks, since early management is relevant to avoid the high burden of morbidity and mortality that it can have if it is not managed in the best way.
