ABSTRACT
Introduction: There are currently no predictive scales, easy to apply, for the failure of non-invasive ventilation in acute chronic obstructive pulmonary disease. Objectives: To validate a predictive scale of failure of non-invasive ventilation in a group of patients based on the factors identified as predictors of failure.
Methods: A cohort study was carried out in patients with acute chronic obstructive pulmonary disease, treated with non-invasive ventilation, graduated from the serious patient care units of the “Saturnino Lora” Provincial Hospital, from January 2011 to January 2016, in whom the predictive factors of failure were identified. The sample was made up of 118 patients. At a second moment, the predictive scale was designed. For its validation, a cohort study was conducted in a new sample of 82 patients with the same characteristics, recruited from January 2016 to January 2019.
Results: The presence of leaks, the onset of symptoms of more than 24 hours, the decrease in the average values of the Glasgow Coma Scale and pH, the increase in heart and respiratory rates and PaCO2, are associated with failure of noninvasive ventilation. The factors with causal magnitude were respiratory rate, heart rate, and PaCO2 at two hours.
Conclusions: The predictive scale of failure of non-invasive ventilation in acute chronic obstructive pulmonary disease is adequate to determine the probability of failure of the therapeutic procedure.
