Third age use of health services

ABSTRACT
Objective: Identify the health services demand by ageing population and determine the morbidity in the group.
Material and methods: A surveillance study in the area of influence of a health center in the City of Nezahualcoyotl was carried out. This Center provides care to a population of 200,000 people from which 2.3% (4,700) of them are older than 65 years. A questionnaire concerning sociodemographic characteristics, morbidity and type of health services demand, was applied to a sample of 300 individuals (confidence level 95% and p ‹ 0.5).
Results: In this group, 70.9% of the sample were women and the age average was 65 years; 50.3% were married, 40% widows and the rest were single or living together. Thirty percent of them were illiterate and 12.9% worked as merchants.
The mode of children per family was nine and 25% of the group still actively work. Twenty percent are pensioned, 11.3% retired and, 46.7% living with their spouse, 42.1% with a son or daughter and 22.2% living alone. Eighty seven point seven percent of them owned a house or have a private bedroom.
Twenty five percent did not have medical service. Regarding to health insurance, 43.3% used IMSS, 16.9% SSA, 13.2% ISSSTE, 0.7% PEMEX or Army Medical Services. Fifteen percent of the users said that quality of the services was bad. Medical services were attended an average of five times during the year. Hospitalization was needed by 14.6% during 1997 and 65.6% went to private medical services. The most frequent illnesses were: diabetes mellitus 26.1%, hypertension 20.9%, arthritis 7.2%, central nervous system diseases 2.6%, respiratory diseases, heart diseases and gastrointestinal diseases in 2.3% respectively.
Conclusions: Although the results of this surveillance can not be applied to the rest of the country, it is worth considering that Nezahualcoyotl City, with and estimated population of two millions, 92,000 will be older than 65 years by this year. and twenty five percent (23,000) of them will be lacking of adequate medical services due to the infrastructure deficiency
