Rhinocerebral mucormycosis associated with diabetic ketoacidosis in a cardiac surgery operated patient

ABSTRACT
Mucormycosis is a serious fungal infection, usually acute onset and often fatal when is affected the rhinocerebral
region in patients with diabetic ketoacidosis or immunocompromised. We report the case of a 30-month-old boy,
diagnosed with a congenital heart disease (tetralogy of Fallot). After his surgery he develop diabetic ketoacidosis
and rhinocerebral mucormycosis.
In immunocompromised patients, there are alterations in the immune system at both the cellular and humoral
levels, with decrease in the neutrophil phagocytic of activity and an opsonization inability. Furthermore, Rhizopus
organisms have an enzyme, ketone reductase, which allows them thrive in acidic environments and high glucose
levels as in patients with diabetic ketoacidosis.
