Antiesophageal reflux surgery

ABSTRACT
A retrospective clinical study to determine efficacy of Nissen funduplicature on patients with hiatus hernia and gastroesophageal reflux was made.
From January1
st 1994 to June 30
th 1996, 88 patients were operated of Nissen Funduplicature; 26 patients were excluded: 12 patients did not accept, 11 patients did not ended medical protocol and three patients died because of causes not related with surgery.
Visick clinical rate (grades I to IV), standardized endoscopy by the Savary-Miller classification (grades I to IV), searching stenosis (grades I to IV), and competence of the funduplicature; gastroesophageal reflux by X-rays, and gammagraphy were also performed in the postoperative period.
Of the 62 patients, 52 patients 83.87% showed competence of the funduplicature by endoscopy, radiology and gammagraphy, with a Visick I classification. Ten patients 16.13% showed alterations, resumed in the next groups; Group I: esophagitis grade IV, infection by
Helicobacter pylori, antral gastritis and reflux. Group II: esophagitis grade III, Barret´s esophagus, chronic pangastritis, antral gastritis, infection by
Helicobacter pylori and reflux. Group III: stenosis grade II, chronic pangastritis, antral gastritis and infection by
Helicobacter pylori.
A statistic significance of P: 0.014 was obtained between the Visick clinical rate and cabinet results.
