1064 nm Nd-yag Q-switched Laser Fragments Calcium Hydroxyapatite Spheres and Accelerates Resolution of Pseudoxanthelasma

ABSTRACT
Background: calcium hydroxylapatite fillers are being used for
treating dark eye bags. In some cases, these injections may result
in the formation of pseudo-xanthelasma lesions which are
difficult to reverse. Recent studies using Q-switched laser on
teeth showed fracture of calcium hydroxylapatite dentine, suggesting
the possibility of its use for removal these compounds
from the skin.
Objective: to evaluate the clinical response of pseudo-xanthelasma
caused by calcium hydroxylapatite fillers to treatment
with Q-switched 1 064 nm laser.
Methods: this case series took place from October 2017 to
December 2019 in a dermatology outpatient clinic. We evaluated
three female patients who were treated with a calcium
hydroxylapatite filler for dark eye bags on their lower eye lid,
whom one to four weeks after the injection developed white
nodules resembling xanthelasma. They received one session of
Q-switched 1 064 nm laser and were evaluated six weeks later
with a photographic follow-up.
Results: all three patients noted significant clearance of
lesions six weeks after treatment with one session of Qswitched
1 064 nm laser, which was noticeable in the photographic
follow-up.
Conclusions: Q-switched 1 064 laser is potentially a novel,
noninvasive treatment for xanthelasma-like lesions secondary
to calcium hydroxylapatite spheres. The possible mechanism
may involve fragmentation of calcium hydroxylapatite spheres
which accelerates reabsorption as they can be more easily engulfed
and removed by macrophages. More studies are needed
to define the role of Q-switched laser for treatment of this
adverse effect on the skin caused by the deposit of injectable
fillers.
