Facial Papules on Five Women with Frontal Fibrosing Alopecia Treated with Oral Isotretionine

ABSTRACT
Facial papules are considered a cutaneous manifestation of
frontal fibrosing alopecia especially in premenopausal women
with higher skin phototypes. Although the precise etiology is
unknown, it has been suggested that there is a structural alteration
of the sebaceous gland, which justifies oral isotretinoin
treatment.
We present five cases of premenopausal Colombian mestizo
women with frontal fibrosing alopecia, eyebrow alopecia,
lichen planus pigmentosum, and facial papules, in whom oral
isotretinoin treatment resulted in a rapid involution of the papules,
with persistence of the other skin findings. Two patients
had a history of dyslipidemia and allergic contact dermatitis.
All biopsy specimens showed alterations in the appearance
and quantity of sebaceous glands, as reported in previous case
series.
Oral isotretinoin is a useful therapeutic alternative in patients
with facial papules and frontal fibrosing alopecia, however,
it does not modify other associated skin lesions.
