Auricular septostomy in a patient with pulmonary arterial hypertension and predictors of high risk transprocedural mortality

ABSTRACT
Introduction: Primary Pulmonary Hypertension is an incurable disease which limits functional capacity in patients. Life expectancy is shorter in those patients. Prostacyclins, prostanoids, endothelin receptors and phosphodiesterase-5 inhibitors improve life quality.
The pathology does not appear in patients who received lung and/or lung-heart transplants. Obliterant brochiolitis and high lethality are serious complications that should be solved in those patients.
Both procedures are not performed currently in Cuba.
Auricular Septostomy is the previous step before surgery in countries in which transplant are available. In our country this procedure is the last step in the treatment of the above-mentioned disease. The Auricular septostomy has an elevated risk of complication and death in this subgroup of patients.
It is intended to present by first time this technique in our country, developed in a patient suffering from Primary Pulmonary Arterial Hypertension with elevated predictors of failure and mortality throughout the procedure. It was performed successfully and free from complications.
Conclusions: Auricular Septostomy is a clear option for Pulmonary Arterial Hypertension’s treatment that improves quality of life among patients. It is recommended to be developed by experts in the field.
