Value of myocardial deformation in the prediction of ventricular dysfunction in mitral regurgitation surgery

ABSTRACT
Introduction: The study of myocardial deformation can be useful to detect latent ventricular dysfunction in patients followed by chronic severe mitral regurgitation.
Objectives: To evaluate the value of the analysis of longitudinal myocardial deformation of the left ventricle as a predictor of medium-term ventricular dysfunction in patients operated on for chronic severe mitral regurgitation.
Methods: An observational, descriptive, prospective longitudinal study was conducted in 74 patients admitted to the Cardiovascular Surgery Service of the Institute of Cardiology and Cardiovascular Surgery in Havana, who underwent surgical treatment for mitral valve insufficiency from January 1, 2014 to January 1, 2016 and divided into two comparison groups according to the presence (n = 23) or not (n = 51) of postoperative ventricular dysfunction.
Results: The left ventricular ejection fraction preoperative (AUC = 0.689) and LV Dp / Dt (AUC = 0.743) were significantly related to a greater probability of postoperative ventricular dysfunction, although with a predictive value lower than deformation parameters. Strain (AUC = 0.855 67.7% sensitivity and 91.2% specificity) and baseline strain rate (AUC = 0.787 58.0% sensitivity and 87.7% specificity) showed a good predictive value of postoperative ventricular dysfunction.
Conclusions: Assessment of systolic function by analyzing myocardial deformation (strain and strain rate) constitutes an accurate and useful method in identifying postoperative ventricular dysfunction in patients operated on for mitral regurgitation.
