Coagulación intravascular diseminada asociada a lesión cerebral grave

RESUMEN
Objetivo: Describir la asociación entre lesión cerebral severa y la coagulación intravascular diseminada (CID).
Diseño: Reporte de casos.
Lugar: UCI de un hospital de tercer nivel de la Ciudad de México.
Pacientes: Cuatro pacientes previamente sanos con lesión cerebral severa después de traumatismo craneoencefálico.
Intervenciones: Se administró plasma fresco congelado, crioprecipitados y plaquetas. A un paciente se le efectuó cirugía de cráneo.
Mediciones y resultados principales: Se observó trombocitopenia, alargamiento del TP, TTP y TT, hipofibrinogenemia, dímero D y productos de degradación de la fibrina elevados. Todos los pacientes murieron y el estudio
post mortem mostró trombos de fibrina en los glomérulos y en la microcirculación pulmonar.
Conclusión: Los pacientes con lesión cerebral severa y CID, tienen mortalidad elevada.
