Neuralgia trigeminal

RESUMEN

La neuralgia del trigémino es una enfermedad incapacitante y misteriosa, que ocurre primordialmente en individuos de la tercera edad. No obstante que esta entidad ha sido estudiada exhaustivamente, aún existen diversas controversias en su etiología y manejo. El Tic Doloreux sintomático se atribuye a un factor mecánico que actúa directamente sobre el trigémino, habiéndose descrito compresiones vasculares y tumorales, o bien, se le asocia a la esclerosis múltiple. La neuralgia trigeminal idiopática constituye un grupo de pacientes de difícil manejo, en los que no se encuentra una causa obvia que produzca el dolor. El cuadro clínico se caracteriza por crisis de dolor intenso, paroxístico, desencadenado por una miríada de factores desencadenantes. La exploración física es negativa. El diagnóstico clínico es fácil de hacer, debiéndose realizar resonancia nuclear magnética para establecer la etiología, y de esta manera normar el tratamiento. La mayoría de los enfermos responden a los antiepilépticos, quedando un reducido grupo que requiere de procedimientos invasivos percutáneos o quirúrgicos.
