El papel del anestesiólogo en cirugía de epilepsia

RESUMEN
La epilepsia es un síndrome que afecta de forma importante la calidad de vida de aquellos pacientes que la padecen. A pesar del tratamiento farmacológico múltiple, 30% no responderá y será candidato para manejo quirúrgico. El manejo anestésico para este tipo de cirugía es un reto para los anestesiólogos, cuyo papel es determinante para el éxito de la cirugía, pues se busca resecar la mayor cantidad de tejido epileptógeno con la mayor conservación de tejido funcional para la vida diaria del paciente. El mapeo es crucial, y éste se realiza con una malla de electrocorticografía para determinar el foco y las áreas de propagación; posteriormente, se determina si estas áreas están involucradas con el movimiento de las extremidades, movimientos finos, lenguaje, abstracción o memoria. Los anestésicos usados de forma habitual pueden afectar el monitoreo, disminuyendo la eficacia del procedimiento y dificultando la localización del foco. El objetivo de esta revisión es puntualizar los aspectos más importantes de la epilepsia que se enfocan al manejo anestésico.
