Preparación del colon en la cirugía colorrectal electiva

RESUMEN
La preparación del colon en la cirugía colorrectal electiva es controversial. Durante las últimas dos décadas, ha surgido evidencia que sugiere que la preparación mecánica por sí sola no ofrece beneficio en cuanto a disminuir el riesgo de complicaciones infecciosas u otras causas de morbilidad, por lo que debe abandonarse. Lo anterior va en contra de lo que se consideraba una práctica de rutina desde hace casi un siglo. Sin embargo, la combinación de antibióticos orales y preparación mecánica ha demostrado ser significativamente efectiva en la disminución de infección del sitio quirúrgico, fuga anastomótica e íleo postquirúrgico, lo que repercute en una menor estancia hospitalaria, readmisión y mortalidad en comparación con el uso de preparación mecánica exclusiva o ninguna preparación. Sin embargo, aún hacen falta estudios con alto nivel de evidencia para establecer esta práctica como una recomendación generalizada. Mientras tanto, la evidencia actual sugiere que debería usarse preparación mecánica en combinación con antibióticos orales para disminuir la morbilidad y mortalidad en estos pacientes.
