Púrpura trombocitopénica trombótica asociada a nefritis lúpica

RESUMEN

Se presenta el caso de un paciente masculino de 34 años, con antecedente de artritis de rodillas tratada previamente con corticosteroides, con buena respuesta. Acudió por edema generalizado, coluria y orina espumosa, malestar general y náusea. Por medio de laboratorios, se encontró trombocitopenia, disminución de la función renal, proteinuria en rangos nefróticos y un sedimento urinario con presencia de cilindros granulosos y eritrocitos. En el estudio etiológico del síndrome nefrótico se encontró hipocomplementemia, anticuerpos antinucleares positivos y anticuerpos anti-ADNdc positivos. Se realizó biopsia renal, donde se halló histopatología compatible con nefropatía lúpica difusa (clase IV-G A8 C2, de acuerdo con la clasificación de ISN/RPS 2003), por lo que se inició un tratamiento de inducción con bolos de metilprednisolona (3) y, posteriormente, ciclofosfamida IV (1 g). Luego se agregó fiebre, cefalea, anemia y trombocitopenia, hiperbilirrubinemia indirecta, disminución de haptoglobinas y un frotis de sangre periférica con esquistocitos. Se llegó al diagnóstico de nefropatía lúpica con púrpura trombocitopénica trombótica. El lupus y la púrpura trombocitopénica trombótica se asocian en sólo 2% de los individuos con lupus y los síntomas pueden ser difíciles de distinguir entre las dos patologías. Se ha descrito la relación entre estos dos padecimientos en forma infrecuente, por lo que el curso natural, el pronóstico y los factores de riesgo para la asociación no se conocen exactamente. La sospecha diagnóstica y la prontitud del tratamiento son fundamentales para el desenlace del sujeto.
