RESUMEN
Históricamente, la capsulitis adhesiva se ha definido como una entidad de etiología desconocida, curso variable, poco predecible y de tratamiento controversial. También se conoce como «hombro congelado», y consiste en la pérdida de movilidad pasiva del hombro, especialmente de la rotación externa, que se acompaña de dolor difuso y limitación funcional. Su curso es insidioso, progresivo y, en la mayoría de los casos, autolimitante. Se presenta con mayor frecuencia entre los 40 y 70 años; es más común en mujeres y pacientes con alteraciones tiroideas o diabetes. El objetivo del tratamiento es restaurar la movilidad articular y la funcionalidad, así como mejorar el dolor mediante el uso de antiinflamatorios, esteroides, terapia física y, en ocasiones, manipulación cerrada bajo anestesia, o bien, liberación capsular por medio de artroscopia. El objetivo de este artículo es revisar todas las definiciones operacionales de la capsulitis adhesiva, describir los agentes causales y cambios patológicos intraarticulares, y exponer las alternativas terapéuticas conservadoras y quirúrgicas para el tratamiento adecuado de quienes la padecen.
