El desafío de la nutrición en el tratamiento de la gastroparesia

RESUMEN
La gastroparesia se define como un retardo en el vaciamiento gástrico en ausencia de
obstrucción mecánica, asociado a síntomas por más de tres meses. El diagnóstico de
elección se realiza mediante la centellografía del vaciamiento gástrico con una comida
radiomarcada, lo que parece ser el estudio más confiable y fisiológico. Una vez
realizado el diagnóstico hay que hacer una evaluación nutricional de los pacientes para
personalizar el tratamiento considerando siempre inicialmente ajustes en la dieta que
podrán aliviar los síntomas en ocasiones sin necesidad de recurrir al tratamiento
farmacológico. En casos excepcionales, en los que la nutrición enteral no logra
administrarse a un ritmo suficiente para mantener el paciente en su rango de peso, o
porque esté afectada la motilidad del intestino delgado, además de la gástrica, puede
ser necesario el uso de nutrición parenteral. En los pacientes diabéticos es
imprescindible un adecuado control glucémico.
