Tuberculoma del seno cavernoso

RESUMEN

En este reporte, se presenta el caso de un paciente masculino de 32 años de edad, que inicia su cuadro clínico con diplopia y dolor facial. Los estudios de imagen iniciales no mostraron alteración alguna y el paciente desarrolló en el curso de los siguientes 12 meses afección completa del III nervio craneal izquierdo. Los estudios subsecuentes mostraron una tumoración en la región del seno cavernoso izquierdo acompañado de edema perilesional en el lóbulo temporal ipsilateral. El paciente fue diagnosticado como un meningioma de la pared lateral del seno cavernoso intervenido quirúrgicamente. El resultado histopatológico fue de un tuberculoma, por lo que recibió tratamiento comple-mentario con rifampicina y etambutol durante 6 meses. Hasta la actualidad el paciente permanece asintomático después de 5 años de seguimiento. Este trabajo se complementó con una revisíón de la literatura acerca de la ocurrencia de tuberculomas a nivel meningeo y en la región del seno cavernoso.
