Tratamiento endoscópico de las estenosis biliares posquirúrgicas: Experiencia de 13 años

RESUMEN
Introducción: Las estenosis biliares posquirúrgicas suelen ocurrir después de una colecistectomía laparoscópica con o sin exploración de la vía biliar en un 0.2 a 0.5%. Con el tratamiento endoscópico mediante la colocación de endoprótesis biliares se obtiene un éxito de resolución de la estenosis mayor al 85%.
Objetivos: Demostrar la seguridad y la eficacia del procedimiento endoscópico en las estenosis posquirúrgicas de la vía biliar e identificar los tipos de lesión de la vía biliar.
Material y métodos: El presente es un estudio descriptivo, retrospectivo y transversal que incluyó la revisión de expedientes de pacientes con diagnóstico de estenosis biliares posquirúrgicas en el periodo comprendido de marzo de 1990 a julio de 2003.
Resultados: Se incluyeron 143 pacientes con diagnóstico de estenosis de la vía biliar: 56 hombres y 87 mujeres, con una edad promedio de 40 años; los tipos de lesiones fueron Bismuth I = 33 pacientes, Bismuth II = 48 pacientes, Bismuth III = 62 pacientes. El 26.5% de los pacientes completaron el protocolo de 2 años; de éstos, en 32 se resolvió la estenosis. Complicaciones: hemorragia = 1, pancreatitis = 2, colangitis = 2 y migración = 6.
Conclusión: La colocación de endoprótesis biliares es el tratamiento de elección en las estenosis biliares posquirúrgicas.
