Remisión a mediano plazo de la diabetes mellitus tipo 2 posterior a cirugía bariátrica en mexicanos: bypass gástrico laparoscópico versus gastroplastia vertical en manga

RESUMEN
Antecedentes: La diabetes mellitus tipo 2 y la obesidad están asociadas, observándose aumento de la prevalencia en mexicanos. La cirugía bariátrica ha demostrado ser el mejor tratamiento para la remisión de la diabetes asociada con la obesidad; actualmente no existen estudios en población mexicana. El objetivo fue analizar y comparar la remisión de diabetes a mediano plazo entre diferentes procedimientos y la pérdida de peso.
Material y métodos: Análisis retrospectivo de 84 pacientes con obesidad y diabetes sometidos a bypass gástrico laparoscópico (n = 62) o gastrectomía vertical en manga (n = 22) entre el año 2009 y 2011 en una sola institución.
Resultados: El 72.6% de los pacientes fueron mujeres. Ambos grupos presentaron un perfil metabólico preoperatorio comparable. Ambos grupos mejoraron los niveles de glucosa y Hb1Ac%, de los 12 hasta los 18 meses sin diferencias significativas. Se observó remisión en el 76.1% de pacientes; completa en 65.4% y parcial en 10.7%, sin diferencias entre grupos. La pérdida de peso fue mayor para el bypass gástrico.
Discusión: Encontramos que ambos procedimientos son igualmente efectivos en hispanos para la remisión a mediano plazo de la DM2, y comparables a lo descrito en otras poblaciones.
Conclusiones: Ambos procedimientos generaron remisión de la diabetes en la población mexicana a mediano plazo. No se observaron diferencias en la remisión parcial y completa entre procedimientos. El bypass gástrico mostró mayor pérdida de peso.
