Ensayo clínico aleatorizado de rivaroxaban en la prevención del síndrome postrombótico

RESUMEN
Antecedentes. El síndrome postrombótico (SPT) es un padecimiento que se presenta hasta en
30% con el tratamiento de enoxaparina y la adición de acenocumarina (E+A). Desde el punto de vista
funcional los pacientes aquejan limitaciones importantes en sus actividades diarias alterando
su calidad de vida. El rivaroxaban (R) ha sido una opción eficaz en su tratamiento. Sin embargo, no
se ha demostrado si puede prevenir el SPT.
Objetivo. Comparar la efectividad del R contra E+A en la prevención del SPT en trombosis venosa
profunda (TVP) femoropoplítea.
Material y métodos. Se incluyeron a pacientes con diagnóstico de TVP femoropoplítea por medio
de ultrasonido Doppler dúplex. Se estableció tratamiento aleatorio con E+A o R. Se valoraron a los
pacientes al mes, tres y seis meses por medio de escala de Villalta y ultrasonido Doppler para verificar
recanalización y reflujo.
Resultados. Ciento sesenta pacientes completaron el estudio. El R redujo el riesgo de SPT (leve o
moderado) en 16.3% (IC95% de 2.3 a 29.4%) al mes, 20% (IC95% de 6.2 a 32.9%) a los tres meses y
de 16.2% (IC95% de 2.9 a 28.9%) en seis meses. Cuando se presentó el SPT en 90% fue leve, el resto
moderado (escala Villalta). Se observó mayor porcentaje de recanalización con el uso de R al
mes, tres y seis meses, con menos incidencia de reflujo.
Conclusión. El uso de rivaroxaban en pacientes con TVP femoropoplítea reduce el riesgo de SPT y
presenta mayor recanalización.
