Embarazo ovárico del segundo trimestre: presentación de un caso

RESUMEN
Antecedentes: el embarazo ectópico ovárico es de baja frecuencia
pero con efectos adversos en la salud reproductiva de la mujer. No
siempre es fácil distinguir si el embarazo es tubárico o abdominal.
Los procedimientos diagnósticos y opciones de tratamiento son un
reto médico.
Caso clínico: paciente de 21 años de edad, con feto vivo de 14
semanas de gestación, valorada en el servicio de Tococirugía como
urgencia médica por abdomen agudo y choque hipovolémico. Se
realizó laparotomía exploradora de urgencia y resección en bloque
de una masa anexial localizada en el lado derecho. Los hallazgos
fueron: hemoperitoneo abundante, con feto vivo en el saco gestacional
íntegro, implantado sobre el ovario derecho. El estudio
histopatológico reportó: embarazo ovárico primario de acuerdo
con los criterios de Spiegelberg. Se discuten aspectos básicos de
la biología del trofoblasto y la asociación dispositivo intrauterinoembarazo
ovárico.
Conclusión: el embarazo ectópico es un problema obstétrico que
incluye aspectos fisiopatológicos poco comprendidos, un dilema de
diagnóstico clínico y un muy pobre arsenal de opciones terapéuticas.
