Daño neurológico secundario a cirugía de revascularización miocárdica con circulación extracorpórea

RESUMEN
Objetivo: describir la incidencia de daño neurológico en pacientes
sometidos a revascularización coronaria con derivación
cardiopulmonar, en el servicio de cirugía cardiotorácica en un
centro cardiológico.
Material y métodos: en forma retrospectiva
fueron seleccionados los expedientes pertenecientes a pacientes
sometidos a revascularización coronaria con circulación extracorpórea,
excluyéndose casos debido a que sus expedientes eran incompletos.
Se analizaron en busca de alteraciones clínicas neurológicas
posteriores a la intervención quirúrgica, valorados mediante
la escala de Glasgow, corroborado por estudios complementarios
y además se obtuvieron datos relacionados a posibles factores de
riesgo. El análisis estadístico de los resultados obtenidos se
presentan en frecuencias, medidas de tendencia central y desviación
estándar.
Resultados: en un año se efectuaron 384 revascularizaciones
miocárdicas de los cuales se excluyeron 44 casos. Se analizaron 340
expedientes y la incidencia de complicaciones neurológicas fue de
2.6% (9 casos de 340), con dos casos (0.5%), catalogados como
lesión grave de los cuales uno falleció. La incidencia y tipo
de complicaciones encontrada coincide con lo reportado; aunque
se tienen limitantes, debido a la perspectiva con la que se
realizó el trabajo (retrospectivo) y que el tipo de evaluación
fue exclusivamente clínico, considerando únicamente lesiones
neurológicas mayores y no déficit neuropsicológicos.
