Manejo de la hemorragia crítica en insuficiencia hepática

RESUMEN
Los pacientes con insuficiencia hepática con frecuencia muestran profundas alteraciones en su sistema hemostático. Mientras que las pruebas de laboratorio de rutina sugieren que estas alteraciones hemostáticas dan como resultado diátesis hemorrágica, la evidencia científica demostrada tanto en estudios clínicos y de laboratorio sugieren que la situación es más compleja. El paciente promedio con insuficiencia hepática puede estar en equilibrio hemostático a pesar de las alteraciones en las pruebas de coagulación, ya que los factores a favor y en contra de la hemostasia se ven afectados; los factores antihemostáticos no son valorables con las pruebas de coagulación de rutina. Esta condición puede plantear dilemas diagnósticos y terapéuticos para los médicos. Con la alta tasa de complicaciones hemorrágicas en pacientes con cirrosis, examinamos los distintos procoagulantes disponibles para su uso en esta población.
