Actualidades en el tratamiento sistémico de la dermatitis atópica en el paciente pediátrico

RESUMEN

La dermatitis atópica es una enfermedad inflamatoria de la piel; tiene una distribución topográfica específica según la edad del paciente y se caracteriza por placas eritematoescamosas y pruriginosas; aparece por brotes; tiene una fisiopatogenia en donde se ve afectada la barrera cutánea, por lo que la base del tratamiento es la educación y el uso de emoliente y esteroides tópicos de baja y mediana potencia; sin embargo, existen formas graves, recalcitrantes o con mala respuesta al tratamiento habitual ante las cuales es necesario el uso de un tratamiento sistémico teniendo como objetivo controlar la enfermedad y mejorar su calidad de vida. Se realiza esta revisión de la literatura con objeto de conocer los fármacos utilizados con mayor frecuencia en nińos con dermatitis atópica severa, las indicaciones que actualmente se conocen, su seguridad al administrarse en población pediátrica, la dosis óptima establecida, los posibles efectos adversos y su seguimiento; sin embargo, queda a criterio del médico tratante individualizar a cada paciente para su manejo y en todos los casos educar sobre la enfermedad y el cuidado de su piel.
