Resultados del tratamiento quirúrgico de 52 casos de anquilosis temporomandibular

RESUMEN

Se realizó un estudio retrospectivo de 52 casos tratados quirúrgicamente de anquilosis temporomandibular fibrosa tipo I y II en 19 pacientes (36.4%) y de tipo óseo, clasificación III y IV en 33 pacientes (63.6%), en 42 niños y adolescentes del Hospital de Pediatría (1983-1985/1989-1998) y del Hospital General de La Raza (1985-1989) y 10 adultos, incluyendo los intervenidos en el Hospital de Especialidades del Centro Médico Nacional Siglo XXI del IMSS, durante el período comprendido de 1998 a 2001. Se utilizó el abordaje preauricular modificado por Dunn en 37 casos (67%) y 17 con la técnica de Risdon submandibular (33%). La causa principal de la anquilosis fueron: los traumatismos directos en mandíbula, que afectaron la zona articular principalmente en los niños, mientras que en el adulto las causas fueron más diversas como la osteomielitis por infección del oído medio, las secuelas de microsomia hemifacial y traumatismos. Los resultados se consideraron como buenos al obtener una apertura bucal de 35 mm, sin recidiva de la anquilosis al año de control postoperatorio.
