Discografía lumbar, controversia y actualidad

RESUMEN
El manejo exitoso del dolor lumbar
requiere de una precisa localización
anatómica de su origen. La discografía
lumbar seguida de una TC, resulta
valiosa para determinar la importancia
de anormalidades discales dudosas o
en múltiples niveles, determinando el
tipo de herniación y definiendo las
opciones quirúrgicas en la evaluación
de columnas previamente operadas.
En un 89-94% de pacientes, la
discografía CT pronostica
correctamente el tipo de herniación
como protrusión, extrusión, secuestro
o disrupción interna. El dato cardinal
es la estimulación discal donde el
dolor provocado determina si el disco
es el origen del dolor. El diagnóstico
y las aplicaciones de la discografía
lumbar siempre serán controversiales
y esta posición no ha cambiado aún
con el advenimiento de la ttomografía
Computada y la Resonancia
Magnética. Esta técnica ha sido
utilizada para el estudio de la
degeneración discal lumbar y ha
proporcionado información no
obtenida por otros medios.
