Puntaje microscópico del sedimento urinario como marcador diagnóstico de lesión renal aguda en sepsis

RESUMEN
La microscopia del sedimento urinario es uno de los marcadores más
utilizados en el diagnóstico de lesión renal aguda; sin embargo, no se
ha validado en lesión renal aguda secundaria a sepsis. Se comunica
el caso de dos pacientes con lesión renal aguda secundaria a sepsis
en quienes se tomaron muestras de orina y se examinó el sedimento
urinario. Evaluamos el puntaje de microscopia del sedimento urinario
con base en la existencia de cilindros granulosos y de células epiteliales
renales. Se realizaron determinaciones plasmáticas de nitrógeno ureico,
creatinina y lipocalina de la gelatinasa del neutrófilo (NGAL). El puntaje
de microscopia del sedimento urinario alto tuvo buena correlación
con otros marcadores de disfunción renal, por lo que consideramos
que la evaluación del puntaje de microscopia del sedimento urinario
en pacientes con lesión renal aguda secundaria a sepsis es una herramienta
valiosa.
