Aneurisma disecante de aorta ascendente. A propósito de un caso y revisión de la literatura

RESUMEN
Los aneurismas aórticos representan la lesión más frecuente y la que más amenaza la vida entre las enfermedades de la
aorta torácica, tienen diferentes factores etiológicos; sin embargo, se ha visto que la asociación con aorta bivalva, en
quienes se han identificado genes responsables de la aparición de aneurismas como NOTCH1, UFDL1, ACTA2, eNOS,
entre otros, aumenta el riesgo de complicaciones como la disección aórtica y posterior ruptura, causando la muerte del
paciente en casi 100%. Se sugiere un patrón autosómico dominante en los casos aislados. Se presenta un caso exitoso
de un paciente de 55 años de edad quien ingresó al hospital por presentar ruptura aneurismática con disección aórtica
que se manifestó como insuficiencia aórtica severa con inestabilidad hemodinámica y taponamiento cardiaco, por lo que
se decidió su tratamiento quirúrgico con el procedimiento Bentall y De Bonno, evolucionando satisfactoriamente.
Al momento de este reporte, el paciente con vida y sin repercusiones en sus actividades cotidianas, por lo que se destaca
la importancia que tiene el diagnóstico oportuno, así como la sincronía de un equipo interdisciplinario en la detección,
tratamiento y posterior atención médica, pues son determinantes para la supervivencia de dicha emergencia que amenaza
la vida de los pacientes
