Papel de la cirugía en pacientes con cáncer de mama metastásico

RESUMEN
El cáncer de mama es la neoplasia maligna con mayor
incidencia en mujeres mexicanas, en un número significativo
de casos se presenta en etapas avanzadas. Las pacientes con
enfermedad metastásica tienen opciones terapéuticas
limitadas, la piedra angular del tratamiento continúa siendo
la quimioterapia sistémica. Tradicionalmente, el papel de la
cirugía en este contexto se ha limitado estrictamente a
la paliación. La mejoría en la eficacia de los agentes
quimioterapéuticos, en conjunto con hormonoterapia y la
terapia blanco molecular, han incrementado la sobrevida de
estas pacientes y expandido las indicaciones quirúrgicas más
allá de la simple paliación. Algunos reportes institucionales
sugieren mejoría en la sobrevida de las pacientes cuando se
logra la resección de enfermedad metastásica limitada.
Asimismo, algunos informes recientes sugieren mejoría del
pronóstico cuando se realiza resección del tumor primario en
presencia de metástasis. Se realizó una revisión sistemática de
la literatura con el objetivo de evaluar el papel de la cirugía en
pacientes con cáncer de mama metastásico y establecer su
lugar en el contexto de manejo multidisciplinario para
mejorar el pronóstico de este grupo numeroso y creciente de
pacientes.
