Fijación vertebral por vía transpedicular. Importancia de los estudios anatómicos y de imagen

RESUMEN
El número de pacientes que padecen patología de la columna vertebral se ha incrementado exponencialmente en los últimos años y con ello, se incrementará el número de casos que requerirán tratamiento quirúrgico. En la actualidad la cirugía de fusión vertebral por vía transpedicular representa la técnica de fijación más comúnmente utilizada para el tratamiento de desordenes vertebrales de cualquier índole. Las características morfométricas de las vértebras, en especial del pedículo, determinan el tamaño de los implantes pediculares tanto en anchura como en longitud, así como la forma, dirección y angulación ideal del tornillo al momento de su introducción. Es importante el conocimiento de estas características para evitar lesiones de estructuras importantes adyacentes, así como para disminuir la incidencia de complicaciones postoperatorias. En las últimas décadas se han efectuado numerosos estudios en distintas poblaciones sobre las características morfométricas del pedículo vertebral para determinar sus dimensiones reales mediante medición directa y métodos de imagen. Estos estudios han llegado a la conclusión de que existen diferencias significativas en estas mediciones entre las diferentes etnias, razas, géneros, edades y regiones vertebrales estudiadas. En el presente artículo se analizan los diferentes estudios morfométricos del pedículo y demás elementos vertebrales estudiados en México y el resto del mundo, justificando la importancia de su conocimiento y aplicación quirúrgica para el correcto desarrollo de la cirugía de fusión vertebral por vía transpedicular.
