Tiroiditis postparto: actualización diagnóstico-terapéutica

RESUMEN
La tiroiditis postparto (TPP) es una enfermedad autoinmune que aparece durante el primer año de la resolución de un embarazo. La prevalencia es variable dependiendo del país y de otros factores; ha sido reportada desde 1.1% hasta 16.7% (media 7.2%) y es más alta en mujeres con diabetes mellitus tipo 1, con historia de un episodio previo de TPP o anticuerpos antitiroperoxidasa positivos. La TPP depende de alteraciones en la inmunidad celular y humoral y hay varios tipos de infiltrados celulares en la glándula tiroides. La presentación clínica es como hipertiroidismo, hipotiroidismo alternando o ambos. Al final del primer año, la mayoría de las mujeres están eutiroideas. Durante la fase de hipertiroidismo, el diagnóstico diferencial es con exacerbación de enfermedad de Graves previa. En la fase de hipotiroidismo puede confundirse con depresión postparto. Se discute el papel de la autoinmunidad en la etiología de esta condición y algunas recomendaciones acerca del diagnóstico, tratamiento, seguimiento y escrutinio. El seguimiento es importante ya que el hipotiroidismo permanente se desarrolla en 30% de estas mujeres en los años siguientes al episodio.
