Esplenectomía en la drepanocitosis

RESUMEN
La crisis de secuestro esplénico es una de las manifestaciones más frecuentes y graves de la drepanocitosis en el niño pequeño. El tratamiento clásico es la esplenectomía total, que tiene el riesgo de sepsis fulminante con una alta mortalidad. Otras complicaciones de la esplenectomía total son el aumento de la incidencia de algunas de las manifestaciones de la enfermedad y desde hace pocos años se vincula con la predisposición a eventos vasculares. En el Instituto de Hematología e Inmunología se realiza la esplenectomía parcial desde el año 1986 con excelentes resultados, pues se elimina casi en su totalidad la recurrencia de los secuestros esplénicos agudos, sin aumentar el riesgo de infecciones. Se ha demostrado que el remanente esplénico conserva su función y se desconoce el riesgo de complicaciones vasculares.
