Tratamiento quirúrgico de la diabetes mellitus tipo 2

RESUMEN
La diabetes mellitus tipo 2 es una enfermedad crónica y progresiva. El tratamiento se ha dirigido a retardar su aparición y disminuir la gravedad de las complicaciones. La cirugía metabólica, una nueva rama de la cirugía bariátrica, ofrece un nuevo horizonte: la remisión completa de la enfermedad. La evidencia científica ha demostrado que ciertas operaciones, en especial la banda gástrica, el
bypass gástrico en Y de Roux y la derivación biliopancreática con cruce duodenal, mejoran el metabolismo de la glucosa. Los estudios clínicos en los que se ha evaluado la repercusión de la cirugía metabólica en enfermos con diabetes mellitus tipo 2 han mostrado mejoría en la función de las células B, sensibilidad y secreción de insulina y reducción en la resistencia en relación con los cambios en la cinética de hormonas que influyen en la saciedad y homeostasis de la glucosa, entre las que destacan: leptina, grelina, adiponectina, péptido 1 semejante al glucagón y polipéptido insulinotrópico dependiente de glucosa. Estos procedimientos se han asociado con remisión de la diabetes mellitus tipo 2 y mejor control metabólico de la enfermedad, lo que los hace una buena alternativa para el tratamiento de la diabetes mellitus tipo 2 en pacientes seleccionados.
