CPAP, a 30 años de su descripción como tratamiento de la apnea del sueño

RESUMEN
El CPAP (presión positiva continua en la vía aérea) es la primera opción terapéutica, y el tratamiento de elección para todo paciente con síndrome de apnea obstructiva del sueño (SAOS) en grado moderado y grave. Es también paso indispensable antes de cualquier recurso terapéutico quirúrgico, y puede utilizarse para predecir la utilidad de los dispositivos de avance mandibular. El CPAP ha demostrado que desaparece la hipersomnia, mejora la calidad de vida, mejora la depresión, corrige la inestabilidad ventilatoria, mejora la función cardíaca, disminuye las cifras de tensión arterial, corrige los niveles de marcadores de inflamación, mejora la sensibilidad a la insulina y disminuye el riesgo de accidentes de tráfico en los pacientes con SAOS. El único problema es que como en todos los tratamientos crónicos, el apego no siempre es el ideal; sin embargo, en el presente trabajo describimos las opciones más actuales para lograr la adherencia al tratamiento con el consecuente beneficio de los enfermos.
