Alteraciones cardiacas en pacientes con lupus eritematoso sistémico y sospecha clínica de miocarditis: diagnóstico por resonancia magnética cardiovascular

RESUMEN
La resonancia magnética cardiovascular (RMC) permite una evaluación integral en pacientes con lupus eritematoso sistémico (LES) y sospecha clínica de miocarditis ya que evalúa parámetros anatómico-funcionales, afectación del pericardio y caracterización del miocardio detectando áreas de inflamación o necrosis, siendo útil para el diagnóstico de miocarditis aguda. El objetivo de este estudio fue identificar alteraciones cardiacas en pacientes con LES y sospecha clínica de miocarditis. Se estudió un total de nueve pacientes portadores de LES, los cuales cumplieron los criterios para este diagnóstico, según el American College of Rheumatology (ACR), con edad promedio de 40.5 años; el 89% fueron mujeres, encontrando disminución de la fracción de eyección del ventrículo izquierdo (FEVI) en el 56% de los pacientes, alteraciones de la contractilidad en el 78%, afectación de alguna de las válvulas cardiacas en el 100%, derrame pericárdico de cualquier grado en el 66%, y lesiones sugestivas de miocarditis en el 44%. La RMC permite en este grupo de pacientes la detección de lesión pericárdica valvular y daño del miocárdico, por lo que este método debe ser considerado ideal para la evaluación y seguimiento de estos pacientes
