Vástagos cementados en la artroplastía de cadera: Historia y evolución

RESUMEN
A pesar de los resultados favorables en la fijación femoral cementada, existe una tendencia creciente al uso de vástagos no cementados. Debido a los resultados exitosos de la fijación acetabular no cementada, su mayor versatilidad y fácil técnica de implantación; a la desinformación sobre la «enfermedad del cemento» y a los malos resultados de los vástagos cementados con superficie rugosa. A pesar de ello, las ventajas de la fijación cementada femoral son numerosas: se puede implementar con éxito en fémures de diversa morfología y calidad ósea; el cirujano puede posicionar el implante con la anteversión femoral deseada, sin que ésta sea determinada por la anteversión natural del fémur; muy baja incidencia de fracturas intra y postoperatorias; permite la adición de antibióticos en pacientes con historia o predisposición a infección y mínimo riesgo de dolor de muslo postoperatorio durante la marcha.
