RESUMEN
El síndrome doloroso regional complejo es un desorden o grupo de desórdenes que pueden desarrollarse como consecuencia de un trauma, con o sin una lesión nerviosa obvia. Se caracteriza por dolor espontáneo, hiperalgesia y alodinia, anormalidades sensoriales, alteración del flujo sanguíneo, de la sudoración y cambios tróficos; así como por un estado agudo con síntomas de inflamación regional y por un estado crónico de desórdenes neuropáticos. El dolor se asocia a los cambios generados por el sistema nervioso autónomo. Las neuronas espinales pueden aumentar su sensibilidad por los cambios anatómicos. A nivel supraespinal se observa reorganización de la corteza somatosensorial. El establecimiento de criterios diagnósticos ha sido difícil dada la variedad del cuadro de esta patología. La inhibición simpática con fentolamina es la prueba más aceptada para diagnosticar un dolor de mantenimiento simpático. Aunque diversas estrategias han sido empleadas para el manejo de síndrome doloroso regional complejo, con ninguna se han obtenidos resultados contundentes.
