La terapia periodontal mediante el principio biológico de la regeneración tisular guiada (rtg) Reporte de un caso clínico

RESUMEN
El objetivo principal de la terapia periodontal ha sido por muchos años, la regeneración del periodonto perdido por efecto de la enfermedad periodontal; pero la mayor parte de los tratamientos se dirigen a la limitación de las secuelas de la enfermedad, más que a la regeneración de los tejidos periodontales, sacrificándose el objetivo principal, ya que se logra un estado clínico de salud caracterizado por un epitelio de unión largo. El propósito de este reporte es presentar los resultados obtenidos por la aplicación del principio de regeneración tisular guiada mediante el empleo de una membrana no absorbible a base de politetrafluoroetileno expandido (PTFE-e), para el tratamiento de una bolsa periodontal de 15 mm de profundidad y pérdida de nivel de inserción de 13 mm. en el central superior izquierdo de un paciente de 26 años de edad con diagnóstico de periodontitis agresiva localizada. Previo tratamiento de conductos del central; el tratamiento quirúrgico periodontal consistió en curetaje por colgajo de la zona, raspado y alisado radicular, la colocación de un injerto de hueso seco descalcificado por congelación (DFDBA), además de colocar una membrana no absorbible de politetrafluoroetileno. Se realizó el seguimiento del caso hasta los 2 años. En la sesión de mantenimiento de los 24 meses se observó radiográficamente, un llenado óseo de 7 mm. con una profundidad al sondeo de 3 mm., y las características clínicas de los tejidos blandos eran las propias de un tejido periodontal en estado de salud. Los resultados obtenidos nos permiten concluir que la regeneración tisular guiada es un método predecible, con muchas expectativas de éxito.
