RESUMEN
Introducción: a pesar de la gran cantidad de información en la literatura médica existe una gran controversia y divergencia de opiniones en el manejo de estas pacientes, por lo que esta revisión pretende ser una guía y contestar las preguntas más importantes concernientes al manejo de la ruptura prematura de membranas pretérmino, de acuerdo con la evidencia científica actual en la literatura.
Objetivo: revisar la literatura médica relacionada con la ruptura prematura de membranas y recomendar el manejo basado en evidencia científica.
Material y métodos: se efectuó una revisión de la literatura en The Cochrane Library y PubMed para la búsqueda de guías de práctica clínica, estudios controlados con asignación al azar, revisiones sistemáticas y meta-análisis, del periodo de 1983 a 2007.
Resultados: las pacientes con ruptura prematura de membranas (RPM), entre la viabilidad fetal y antes de las 32 semanas, deben manejarse de forma expectante si no existen contraindicaciones maternas o fetales. Se deben manejar con un curso único de esteroides y antibióticos por 7 días.
Conclusiones: esta Guía de Práctica Clínica permite plasmar consenso en el manejo de la ruptura prematura de membranas y efectuar recomendaciones para disminuir la variabilidad de la atención y mejorar la calidad de la práctica clínica.
