Lesiones cardiacas penetrantes. Una revisión desde sus orígenes históricos hasta las últimas fronteras en el nuevo milenio. Parte I

RESUMEN

Objetivo: Describir los aspectos en el manejo de las lesiones cardiacas, desde su historia hasta las últimas vertientes del pensamiento quirúrgico y su influencia en el manejo de las mismas.
Diseño: Revisión y selección de 89 referencias bibliográficas.
Sede: Hospital de tercer nivel de atención, universitario. Nivel I de Colegio Americano de Cirujanos.
Resultados: Desde el inicio de la humanidad, hasta la primera reparación con éxito (supervivencia) de una herida cardiaca, por Rehn, a finales del siglo XIX en Alemania, hecho que marcó el inicio de la cirugía cardiaca, pasaron miles de años. El propósito de este trabajo es realizar un recorrido a lo largo de la historia del manejo de estas lesiones, desde las primeras importantes descripciones de las exanguinantes y siempre mortales lesiones cardiacas, aparecidas en la Iliada, hasta una detallada enumeración de los signos y síntomas que nos llevarán, hoy en día, a crear un diagnóstico de certeza, o de sospecha; y en estos casos, los métodos de evaluación más actuales, como la realización de una ventana pericárdica subxifoidea (descrita por primera vez por Larrey) o una ecografía bidireccional (método no invasor de primera elección en los centros de trauma), las indicaciones de cada método, su realización y la interpretación de los resultados obtenidos. Los autores, en las puertas del nuevo milenio, pretenden realizar una extensa revisión sobre el tema, a la vez rendir un homenaje a todos los autores, a lo largo de la historia, que han aportado su esfuerzo y conocimiento, para que hoy la cirugía cardiaca sea una realidad. Los autores aportan su experiencia en el manejo de estas lesiones.
